Weber's syndrome, also known as midbrain stroke syndrome or superior alternating hemiplegia, is a form of stroke that affects the medial portion of the midbrain. It involves oculomotor fascicles in the interpeduncular cisterns and cerebral peduncle so it characterizes the presence of an ipsilateral lower motor neuron type oculomotor nerve palsy and contralateral hemiparesis or hemiplegia.
Wallenberg syndrome was initially described by Gaspard Vieusseaux of Geneva in 1810. It was characterized by vertigo, unilateral facial numbness, loss of pain and temperature appreciation in the opposite limbs, dysphagia and hoarseness, minor tongue involvement, hiccups and a dropped eyelid [1].
It’s often caused by a stroke in the brain stem — the We experienced a malnourished patient with Wallenberg syndrome (lateral medullary syndrome) whose swallowing‐related muscle mass was shown to improve on ultrasound following aggressive nutritional therapy used to improve malnutrition. 2 CASE. The patient was a 62‐year‐old man admitted to the hospital with dysphagia and ataxia. 2021-03-06 Waardenburg syndrome is a group of rare genetic conditions characterised by at least some degree of congenital hearing loss and pigmentation deficiencies, which can include bright blue eyes (or one blue eye and one brown eye), a white forelock or patches of light skin. These basic features constitute type 2 of the condition; in type 1, there is also a wider gap between the inner corners of the Start studying USMLE Neurology- Lesions and Stroke Syndromes. Failure to convey oxygen in the blood to this area causes ischemia (lack of blood in tissues). A 65-year-old white man diagnosed with right superior lateral medullary syndrome first presented for assessment of dysphagia 12 weeks after the initial neurological event.During this 12-week period, the patient underwent a stormy course with aspiration pneumonia requiring a tracheostomy and insertion of a percutaneous gastrostomy tube.On examination, the typical neurological findings of Wallenberg syndrome results from a CVA in the vertebral or posterior inferior artery of the cerebellum and manifests as various degrees of cerebellar dysfunction. The administration of a thrombolytic medication has been recommended in the treatment of patients with stroke caused by cervical artery dissection.

Medial medullary syndrome, also known as inferior alternating syndrome, hypoglossal alternating hemiplegia, lower alternating hemiplegia, or Dejerine syndrome, is a type of alternating hemiplegia characterized by a set of clinical features resulting from occlusion of the anterior spinal artery. This results in the infarction of medial part of the medulla oblongata 2016-07-19 Lateral medullary syndrome, also known as Wallenberg syndrome, is a clinical syndrome caused by an acute ischemic infarct of the lateral medulla oblongata. This is most commonly due to occlusion of the intracranial portion of the vertebral artery followed by PICA and its branches 1-3. Wallenberg syndrome also known as lateral medullary syndrome or the posterior inferior cerebellar artery syndrome, is a neurological disorder is associated with a variety of symptoms that occur as a result of damage to the lateral segment of the medulla posterior to the inferior olivary nucleus 1). Weber syndrome is a midbrain stroke syndrome that involves the cerebral peduncle and the ipsilateral fascicles of the oculomotor nerve 1-3,5. Occasionally the substantia nigra can also be involved 5.

Spinocerebellar tract. Posterior spinocerebellar tract: Ascends and enters to ipsilateral cerebellum via ipsilateral inferior cerebellar peduncle Anterior spinocerebellar tract: Ascends and enters to ipsilateral cerebellum via superior cerebellar peduncle (although it crosses to opposite side on spinal cord along with antero-lateral system, it crosses back to the same side in cerebellum)
According to the National Institute of Neurological Disorders and Stroke, Wallenberg Syndrome (aka Lateral Medullary Syndrome or Posterior Inferior Cerebellar Artery Syndrome) is a neurological condition caused by a blockage of the vertebral artery (VA) or posterior inferior cerebellar artery (PICA), ultimately leading to infarction of the lateral medulla. Lateral Medullary Syndrome (Wallenberg): Contralateral hemibody pain/temp loss [Ipsilateral CN 9, 10] Ipsilateral dysphagia (hoarsness) loss gag reflex vertigo diplopia and nystagmus Ipsilateral Horner's Syndrome Ipsilateral loss of facial pain and temperature Ipsilateral ataxia
Wallenberg Syndrome is when you damage blood supply to the LATERAL medulla (i.e. PICA) 1) You damage your Nucleus ambigus: pt will complain of dysphasia, dysarhria 2) Damage the Trigeminal nerve (note the trigeminal nucleus is in the pons but the pathway continues down the spinal cord so you'll find some fibers in the medulla laterally).
